ThinkGlucose: Inpatient Care for People with Diabetes

More than 20% of inpatients at Guy’s and St.Thomas’ NHS Foundation Trust have diabetes, which puts them at higher risk for complications during their stay in hospital. Guy’s and St.Thomas’ have adopted the ThinkGlucose toolkit to help ward staff manage patients’ blood glucose and act appropriately.

This resource includes a short video introducing the toolkit and demonstrating how to use the blood glucose monitoring chart and guidelines. This is followed by a guided tutorial to practice using the toolkit, which leads to a personalised certificate of completion.

The toolkit was initially designed by the NHS Institute for Innovation and Improvement and adapted for Guy’s and St.Thomas’ by the Safety and Quality in Diabetes group. After launching  the toolkit to frontline clinical teams and patients at Guy’s and St.Thomas’, there was a  marked decrease in medication errors, and improvements in blood glucose documentation and management.
